Alcohol reaches nearly every organ. Its complications arrive quietly - often after years - and several are irreversible once established.
Alcohol reaches nearly every organ. Its complications arrive quietly - often after years - and several are irreversible once established. This is the honest ledger, and the reason early treatment matters so much.
Alcohol is a proven carcinogen: mouth, throat, oesophagus, liver, breast, and bowel cancers all rise with intake, per NHS alcohol misuse guidance. Add infertility, sexual dysfunction, bone thinning, accidents, and the social wreckage - debt, lost work, broken families.
Madhava Nidana (Chapter 18) and Charaka catalogued Madatyaya's Upadrava (complications): severe burning and fever, vomiting of blood, tremors, locked jaw, abdominal distension, wasting, deep unconsciousness - and marked the presentations with discoloured tongue, lips, and nails as Asadhya (beyond treatment), requiring urgent physician care. The classical authors could not name cirrhosis or cardiomyopathy, but they drew the correct operational line centuries early: advanced drinkers' disease is an emergency room problem, not a home-remedy problem.
Which complications reverse if I stop? Fatty liver, gastritis, blood pressure, sleep, and early neuropathy improve substantially with abstinence. Cirrhosis and established Korsakoff's do not reverse - stopping then is about preventing worse.
How much drinking causes cirrhosis? Risk rises with dose and years; heavy daily drinking over a decade-plus is the classic path, but individual susceptibility varies widely - there is no safe heavy level.
Can the brain recover? Much of the fog lifts within months of abstinence with good nutrition. Established Korsakoff's memory loss is largely permanent - a key reason thiamine is given urgently in treatment.
Are the classical descriptions reliable? As observation, remarkably so - the texts recognised the emergency signs. As mechanism, they are interpretive. Modern imaging, endoscopy, and labs define the actual damage.

Several medicines have solid evidence in alcohol use disorder.
Alcohol use disorder announces itself gradually. The early signs are easy to explain away; the late ones are hard to reverse.
"Alcoholic" is not one thing. Modern medicine describes a spectrum, and catching a pattern early on that spectrum is far easier than treating it late.
No single cause explains alcoholism. Alcohol boosts the brain's calming and reward signals.
The shortest honest answer: earlier than feels necessary. Alcohol problems reward waiting with worse odds.
Alcohol use disorder is treatable at every stage, and treatment follows a clear sequence.
