The usual course of a sore throat and patterns that need a doctor.
A typical sore throat from an infection follows a short, predictable course. Knowing the usual stages helps you know when symptoms are not following the normal pattern.
Stage 1: Irritation (day 1) A scratchy, tickling or dry feeling in the throat, often with tiredness or a mild fever. Swallowing may start to feel uncomfortable.
Stage 2: Peak symptoms (days 2 to 3) Throat pain is at its worst, with painful swallowing, redness and possibly swollen glands. Fever, headache, body aches, runny nose or cough may be present. In strep throat, white patches may appear on the tonsils.
Stage 3: Recovery (days 4 to 7) Pain and fever settle, and swallowing becomes comfortable. A mild cough or hoarse voice may linger for a few more days.
Most viral sore throats resolve within 7 to 10 days. Strep throat improves within a day or two of starting antibiotics when they are needed.
When the pattern is different
These patterns need a doctor to look for complications or another cause.
Ayurvedic view (interpretive) Texts describe stages of dosha aggravation and settling in throat disease. This is a conceptual framework only.
When to see a doctor Worsening after day 3, high or lasting fever, one-sided swelling, or symptoms beyond a week. Call 112 for breathing difficulty or drooling.
Remember A typical sore throat peaks by day 3 and clears within a week. Anything outside this pattern needs a check.
