Sore throat Home Remedies and the Ayurvedic View

Sore Throat

Sore Throat is a common condition characterized by pain, irritation, or scratchiness in the throat, often worsened by swallowing. In Ayurveda, throat disorders fall under the category of Kantha Roga (Diseases of the Throat) or Mukha Roga (Diseases of the Oral Cavity). A specific and severe form of throat inflammation described in classical texts is Rohini, which involves painful fleshy growths or inflammation in the throat. General throat pain is also referred to as Kantha Shula.

1. Definition

Modern Definition: Inflammation of the pharynx (pharyngitis) or tonsils (tonsillitis) causing pain and difficulty in swallowing.
Ayurvedic Definition: A condition where the Doshas (bio-energies) vitiate the flesh and blood in the throat, producing fleshy sprouts, swelling, and intense pain that can obstruct the passage of food and air.

2. Etymology & Conceptual Meaning

• Kantha: Means "Throat."
• Roga: Means "Disease."
• Rohini: Derived from the root meaning "that which grows" or "ascends." It refers to a condition where fleshy sprouts grow in the throat, causing obstruction.
• Conceptual Interpretation: Ayurveda views the throat as a vital passage (Prana-vaha and Anna-vaha srotas). When the Doshas (Vata, Pitta, Kapha) and Rakta (blood) become aggravated, they lodge in the throat tissues, causing inflammation (Shotha) and pain (Shula).

3. Causes (Nidana)

The Madhava Nidana outlines specific dietary and lifestyle factors that lead to diseases of the mouth and throat.

• Dietary Causes:
  • Excessive consumption of Matsya (fish) and Mahisha (buffalo milk products like heavy curd/yogurt).
  • Intake of Varaha (pork) and Amaka (uncooked radish or raw meats).
  • Consumption of Masha (black gram) and Supyus (soup of pulses).
  • Eating Pistanna (heavy flour preparations) and Dadhi (curd/yogurt).
  • Excessive intake of Sukta (sour/fermented vinegar) and Madya (alcohol).
• Lifestyle Causes:
  • Divaswapna: Sleeping during the day.
  • Avakshayina: Sleeping in a prone position (face down).
  • Durvamana: Improperly induced vomiting or suppression of the urge to vomit.

4. Journey of the Disease (Pathogenesis/Samprapti)

• Samprapti Ghataka (Components):
  • Dosha: Kapha and Pitta are often dominant, with Vata causing pain. Rakta (Blood) is significantly involved in severe inflammation.
  • Dushya (Tissues): Mamsa (Flesh/Muscles) and Rakta (Blood).
  • Srotas (Channels): Annavaha (food carrying) and Pranavaha (breath carrying) channels.
  • Agni: Mandagni (weak digestive fire) leading to Ama (toxins).
  • Adhisthana (Site): Kantha (Throat) and Gala (Neck region).
• Pathogenesis:
  1. Due to the etiological factors (like heavy, slimy foods), the Doshas (specifically Vata, Pitta, Kapha) become aggravated.
  2. They move upwards (Urdhwaga) and lodge in the throat (Kantha).
  3. They vitiate the local flesh (Mamsa) and blood (Rakta).
  4. This leads to the formation of fleshy growths (Ankura), swelling, and inflammation known as Rohini, which obstructs the throat.

5. Symptoms (Lakshana)

The symptoms vary based on the specific type of throat disorder:

• General Symptoms:
  • Kantha Shula: Pain in the throat.
  • Galagraha: Difficulty in swallowing or "catching" of the throat.
  • Shotha: Swelling.
  • Lalapraseka: Excessive salivation.
• Advanced Symptoms (Rohini):
  • Vataja: Severe pain (Vedana) all around the tongue and throat, obstruction of the passage.
  • Pittaja: Burning sensation (Daha), rapid suppuration (pus formation), and high fever (Tivra Jwara).
  • Kaphaja: Heavy, fixed, nodular swelling with mild pain, blocking the throat passage.
  • Tridoshaja: Manifests with deep-seated suppuration and is incurable.

6. Different Types or Classification (Bheda)

Madhava Nidana classifies throat diseases (Kantha Roga) into 18 types, including:

1. Rohini: (5 types: Vata, Pitta, Kapha, Tridosha, Rakta) - Severe inflammation akin to Diphtheria.
2. Tundikeri: Enlargement resembling the cotton plant fruit (Tonsillitis) characterized by a hard, thick swelling.
3. Kantha Shaluka: A knotty tumor/growth in the throat caused by Kapha (Adenoids/Polyp).
4. Galavidradhi: Abscess in the throat (Peritonsillar/Retropharyngeal abscess).
5. Swarabheda: Hoarseness of voice, which is classified into 6 types based on Doshas.

7. Diagnosis

• Ayurvedic Diagnosis (Roga Pariksha):
  • Darshana (Observation): Inspecting the throat for redness (Raga), swelling (Shotha), or fleshy growths (Ankura).
  • Prashna (Questioning): History of dietary habits (curd, fish) and nature of pain (pricking vs. burning).
• Differential Diagnosis:
  • Distinguished from Mukha Paka (Stomatitis) which affects the mouth lining, whereas Kantha Roga is localized to the throat.
  • Swarabheda (Hoarseness) is diagnosed separately if the primary symptom is only change in voice without significant pain or swelling.

8. Disease Management

Management involves Shodhana (purification) and local treatments like gargling.

Diet (Ahara)

• Pathya (Beneficial): Light foods, Barley (Yava), Green gram (Mudga), and warm water.
• Apathya (Avoid): Curd/Yogurt, milk products, heavy meats, day sleep, and sour foods.

Medicines

• Khadiradi Gutika: A specific pill meant to be kept in the mouth/throat. It treats diseases of the teeth, throat, and mouth, including anorexia and lockjaw. It contains Khadira, Arimeda, Candana, Padmaka, and Triphala.
• Talisadi Churna: Indicated for Kasa (cough), Svasa (asthma), and throat disorders (Kantha Roga).
• Sitopaladi Churna: Often used with honey and ghee for burning sensation in palms/soles and respiratory/throat issues.
• Vyaghri Haritaki: A formulation used for throat and cough issues.
• Lavangadi Churna: Containing cloves, used for Kasa (cough) and throat irritation (Svarabheda).

Local Therapies (Kriya Kalpa)

• Gandusha (Holding liquid) & Kavala (Gargling):
  • Gargling with decoctions of Triphala, Trikatu (ginger, pepper, long pepper), and salt is recommended to clear Kapha and reduce swelling.
  • Tila-adi Gutika: A paste of sesame seeds, lotus stalk, and sugar taken with milk for throat soothing.
• Pratisarana (Rubbing): Application of medicinal powders (like Trikatu mixed with honey and oil) directly on the tonsils or throat swelling.
• Nasya: Nasal administration of medicated oils to clear the head and neck channels.

9. Prognosis

• Sadhya (Curable): Conditions caused by a single Dosha (Eka-doshaja) and treated early are generally curable.
• Asadhya (Incurable):
  • Tridoshaja Rohini: When all three doshas are involved, the condition is considered deep-seated and incurable.
  • Raktaja Rohini: Throat conditions involving vitiated blood with blisters resembling burning coal are incurable.
  • Galaugha: Severe obstruction where the patient cannot swallow water or breathe is fatal.

10. Modern Correlations

• Pharyngitis: Correlates with general Kantha Shula or Vataja Rohini.
• Tonsillitis: Directly correlates with Tundikeri (hard swelling in the throat).
• Diphtheria: Strongly correlates with Rohini, due to the description of "fleshy sprouts" or membranes obstructing the throat and its potential fatality.
• Peritonsillar Abscess: Correlates with Galavidradhi.

12. FAQ Section

Q: Is curd (yogurt) bad for a sore throat?
A: Yes. According to Madhava Nidana, excessive consumption of Dadhi (curd) is explicitly listed as a causative factor (Nidana) for throat and mouth diseases (Mukha Roga) because it increases Kapha and Pitta.

Q: What is the best Ayurvedic pill for sore throat?
A: Khadiradi Gutika is a classical formulation widely prescribed for throat diseases, dental issues, and mouth ulcers. It is kept in the mouth and slowly dissolved.

Q: Can sleeping during the day cause throat problems?
A: Yes. Divaswapna (sleeping during the day) is mentioned as a lifestyle cause for throat disorders as it aggravates the Kapha dosha, leading to congestion and swelling in the throat region.

Q: What is "Rohini" in Ayurveda?
A: Rohini is a severe type of throat disease characterized by painful fleshy growths or severe inflammation that can obstruct the throat. It is often correlated with Diphtheria or severe bacterial pharyngitis/tonsillitis. If it involves all three doshas, it is considered incurable.
