Smallpox Causes and Symptoms

2. Etymology & Conceptual Meaning

• Literal Meaning: The word Masurika is derived from Masura, which means "Lentil" (specifically red lentils or Lens culinaris).
• Conceptual Interpretation: The disease is named so because the shape, size, and often the color of the skin eruptions (Pidaka) closely resemble a lentil seed.

3. Causes (Nidana)

The causes are a mix of dietary indiscretions, environmental factors, and contagion.

• Dietary Causes:
  • Excessive consumption of foods that are Pungent (Katu), Sour (Amla), and Salty (Lavana).
  • Viruddha Ahara: Eating incompatible food combinations (e.g., fish with milk).
  • Consumption of grains that have been corrupted by pests or are of low quality.
• Environmental & External Causes:
  • Grahadosha: Ancient texts attribute the cause to planetary or unseen malignant influences, which parallels the modern understanding of viral/invisible pathogens.
  • Pollution: Vitiation of air (Vayu) and water (Jala).
• Contagion: It is explicitly listed as a disease that spreads from person to person through contact (Gatrasamsparsha) and breath (Nishvasat).

4. Journey of the Disease (Pathogenesis/Samprapti)

• Samprapti Ghataka (Components):
  • Dosha: Tridosha (Vata, Pitta, Kapha) are all involved, often with a dominance of Pitta and Rakta (blood).
  • Dushya (Tissues): Rakta (Blood), Twak (Skin), Mamsa (Flesh).
  • Adhisthana (Site): Whole body, specifically skin and blood vessels.
• Pathogenesis:
  1. Due to the intake of improper diet or environmental contagion, the Doshas become aggravated.
  2. These Doshas enter the Rakta (blood) and circulate throughout the body.
  3. They lodge in the Twak (skin) and Mamsa (flesh).
  4. The vitiated blood and heat generate pustules (Pidaka) that look like lentils.

5. Symptoms (Lakshana)

• Prodromal Symptoms (Purvarupa - Early Signs):
  • Jwara: High Fever.
  • Kandu: Itching.
  • Gatrabhanga: Severe body pain or breaking pain in limbs.
  • Twak-vaivarnya: Discoloration of the skin.
  • Netraraga: Redness of the eyes.
• Advanced Symptoms:
  • Eruption of pustules which may ripen, ooze, and form crusts.
  • Daha: Intense burning sensation.
  • Shotha: Swelling/Inflammation.

6. Different Types or Classification (Bheda)

Madhava Nidana classifies Masurika into five types based on the dominant Dosha:

1. Vataja: Pustules are blackish/dark, dry, rough to touch, and associated with intense pain.
2. Pittaja: Pustules are red, yellow, or white, accompanied by severe burning, suppuration (pus), and pain.
3. Kaphaja: Pustules are white, oily/unctuous, large, very itchy, and dull in pain.
4. Raktaja (Blood-born): Pustules resemble coral (Pravala), are very red, and are considered fatal.
5. Sannipataja: Caused by all three Doshas; pustules are flat in the center, difficult to ripen, and accompanied by delirium and extreme restlessness.

7. Diagnosis

• Ayurvedic Diagnosis (Roga Pariksha):
  • Darshana (Observation): Identifying the characteristic "lentil-like" shape of the boils.
  • Sparshana (Touch): Checking for heat (fever) and the texture of the pustules (rough in Vata, oily in Kapha).
  • Ashtavidha Pariksha: Examination of pulse (Nadi), urine, and feces to determine the Dosha dominance.
• Differential Diagnosis:
  • Distinguished from Visaphota (blisters) and Romantika (Measles). Laghu Masurika is the term often used for the milder Chickenpox.

8. Disease Management

The treatment principle follows the management of Kushtha (skin diseases) and Visarpa (erysipelas/herpes).

Medicines

• Decoctions: Patoladi Kwatha (Patola, Neem, etc.) and Nimbadi Kwatha are prescribed to purify blood and reduce Pitta.
• Khadira & Nimba: Essential herbs for destroying skin pathogens and toxins.
• Pitta-Shamaka: Cooling remedies are prioritized to handle the fever and burning sensation.

Panchakarma (Therapies)

• Vamana (Emesis): In early stages dominated by Kapha, mild emesis may be used (caution advised).
• Virechana (Purgation): To expel Pitta and purify the blood.
• Lepa (External Application): Pastes made of herbs like Neem, Turmeric, and Sandalwood are applied to soothe itching and burning.

Diet (Ahara)

• Pathya (Beneficial): Light diet, bitter vegetables (like Bitter Gourd/Karela, Pointed Gourd/Patola), and green gram soup.
• Apathya (Avoid): Sour, salty, pungent foods, heavy meats, and dairy combinations.

9. Prognosis

• Sadhya (Curable): The Vataja, Pittaja, and Kaphaja types are generally curable if treated early.
• Asadhya (Incurable):
  • Raktaja Masurika: Cases where pustules look like coral and are associated with severe bleeding are considered fatal.
  • Complications: If the patient develops cough, hiccup, delirium, and restlessness, the prognosis is poor.

10. Modern Correlations

• Smallpox: The classical descriptions of Masurika (especially the fatal Raktaja type) correlate historically with Smallpox.
• Chickenpox: The term Laghu Masurika ("Light" Masurika) is correlated with Chickenpox (Varicella Zoster), which is milder.
• Viral Etiology: The Ayurvedic concept of Grahadosha (invisible external agents) and Aupasargika (contagion) aligns with the viral transmission of poxviruses.

12. FAQ Section

Q: Is Masurika contagious?
A: Yes. Ayurveda classifies Masurika as an Aupasargika Roga (contagious disease). It spreads through physical contact (Gatrasamsparsha) and sharing breath/being in close proximity (Nishvasat).

Q: Why is it called "Masurika"?
A: It is named after the lentil (Masura or Masoor Dal) because the pustules or boils that appear on the skin resemble the shape and often the color of lentils.

Q: What diet is best during this disease?
A: A diet that pacifies Pitta and Kapha is recommended. This includes light foods, bitter vegetables like Neem and Patola (pointed gourd), and avoiding salt, sourness, and heavy oils.

Q: Does Ayurveda differentiate between Smallpox and Chickenpox?
A: Yes. While the classical term Masurika historically covered Smallpox, modern Ayurvedic practitioners and texts often use the term Laghu Masurika (Light Masurika) to describe Chickenpox, distinguishing it from the more severe forms.
