Grey throat membrane that bleeds, a swollen bull neck, hoarseness and noisy breathing - then the toxin attacks heart and nerves.
Symptoms usually begin two to five days after infection, often mildly, which is part of what makes diphtheria dangerous - it can look like an ordinary sore throat at first.
Early symptoms:
The hallmark sign is the pseudomembrane: a thick, grey or grey-green coating that spreads over the tonsils, the back of the throat and sometimes the soft palate. Unlike the white patches of a simple tonsillitis, this membrane sticks tightly and bleeds when scraped. It can extend into the airway and block breathing.
Other classic signs:
Get emergency help immediately - call 102 or 112 - if a child with a sore throat has any grey coating in the throat, a visibly swollen neck, noisy breathing, drooling or obvious distress.
Later symptoms come from the toxin, not the throat. As it circulates, it can cause:
Skin diphtheria, seen mostly in tropical settings, causes slow-healing ulcers with a greyish membrane, usually on the legs.
Any suspected case is a medical emergency. Do not wait for all these signs to appear.

Diphtheria is a vaccine-preventable bacterial infection whose toxin attacks the throat, heart and nerves - and it is an emergency.
Soft, cool, calorie-dense food and constant sips of fluid - feeding through a raw throat and palatal paralysis, then rebuilding strength.
Suspect it, treat it, confirm later - diphtheria antitoxin cannot wait for swab cultures, PCR and Elek tests.
A toxin-making bacterium, spread by coughs and close contact - and almost every case traces back to a missed vaccination.
Nasal, tonsillar and laryngeal diphtheria, skin diphtheria and the malignant form - location decides the danger.
Pentavalent at 6-10-14 weeks, boosters at 16-24 months, 5 years and Td at 10 and 16 - vaccination is the only prevention.
