Exercise therapy, DMARDs, urate-lowering and joint replacement: how each type of arthritis is actually treated.
Arthritis treatment is type-specific - the plan that transforms rheumatoid arthritis does nothing for gout, and osteoarthritis care is its own discipline. But across all types the architecture is the same: confirm the type, treat with the therapies that change the disease (not just the symptoms), and build the daily habits that protect joints for the long run, per NHS guidance on arthritis.
The modern plan, in order of importance:
Treat early, treat to target. DMARD therapy - methotrexate first - started in the first months and adjusted until inflammation is suppressed to remission or near it. Biologic and targeted drugs for those who need more. Steroid bridges for flares. Regular monitoring of blood tests, disease activity, and the heart, lung and bone health RA can affect. With modern therapy started promptly, most people with RA work, move and live without deformity. The treatment is lifelong but the life is full.
Two commitments: relieve attacks fast (NSAID, colchicine or steroid at the first twinge) and lower uric acid for good (daily allopurinol or equivalent to a target level, with dose adjusted over months). Do both and gout becomes a cured problem for most people - attacks stop, tophi dissolve, joints are saved. Do only the first and attacks escalate for decades.
Good arthritis care is a team sport: the GP or rheumatologist leads, physiotherapists rebuild strength and movement, occupational therapists joint-proof daily life, dietitians handle weight and gout patterns, and surgeons enter when replacement is the honest answer. Classical Ayurvedic care - oil therapies, joint-nourishing herbs, digestive support - can accompany all of this as a comfort layer under the rules in the Ayurvedic treatment article: disclosed to the team, never delaying DMARDs or urate therapy.
Can arthritis be cured completely? Gout, effectively yes - with sustained urate-lowering treatment. RA and OA are controlled rather than cured: RA can reach drug-maintained remission, and OA symptoms can become minimal with the right programme. "Well controlled" is a normal, achievable outcome.
When is joint replacement the right call? When end-stage joint damage limits your life despite proper exercise therapy, weight management and medication - decided with an orthopaedic surgeon. For the right person it is transformative, not desperate.
How long does DMARD treatment last in RA? Usually long-term - RA is a chronic condition, and stopping effective therapy invites relapse. Some people in deep, sustained remission can cautiously taper under rheumatology supervision. Never stop on your own.
What is the biggest treatment mistake people make? Treating pain while the disease marches on: painkillers for RA without DMARDs, flare relief for gout without lowering uric acid, or years of enduring OA without ever doing the exercise therapy. Symptom relief is the passenger; disease treatment is the driver.
