Schizophrenia Symptoms and Causes

Schizophrenia

Schizophrenia is a chronic and severe mental disorder affecting how a person thinks, feels, and behaves. In Ayurveda, this condition correlates with Unmada (Insanity/Psychosis). It is classified as a disorder of the mind (Manas) characterized by the perversion of intellect (Buddhi), mind (Manas), and memory (Smriti), leading to erratic behavior and a loss of contact with reality.

1. Definition

Modern Definition: A serious mental condition involving a breakdown in the relation between thought, emotion, and behavior, leading to faulty perception (hallucinations), inappropriate actions, and withdrawal from reality.

Ayurvedic Definition: Unmada is defined as the confusion or perversion of the Mind, Intellect, Consciousness, Memory, Desire, Conduct, and Behavior. It represents a state where the doshas (bio-energies) afflict the mind, causing the person to lose control over their mental faculties and act against social norms.

2. Etymology & Conceptual Meaning

• Unmada: The term implies a state of high mental agitation or intoxication.
• Conceptual Interpretation: Ayurveda views the Hridaya (Heart) as the seat of consciousness (Chetana) and the mind. In Unmada, the vitiated Doshas travel from their physical seats, enter the heart, and obstruct the Manovaha Srotas (channels of the mind). This obstruction clouds the intellect (Buddhi), causing the person to forget true knowledge and social conduct.

3. Causes (Nidana)

The causes are a mix of biological, dietary, and psychological factors that vitiate the Doshas and the mind.

• Dietary Causes:
  • Consumption of Viruddha Ahara (incompatible foods) and food that is unclean or improperly prepared.
  • Use of substances that have qualities similar to poison or intoxicants that agitate the mind.
• Lifestyle Causes:
  • Prajnaparadha (Intellectual Error): This is considered the root cause of all disorders. It includes acting against one's better judgment, disrespecting elders/teachers, and improper bodily activities.
  • Suppression of natural urges or forcible propulsion of urges.
• Psychological Factors:
  • Excessive fear (Bhaya), grief (Shoka), anger (Krodha), or excessive joy (Harsha).
  • Mental shock or trauma (Manobhighata).

4. Journey of the Disease (Pathogenesis/Samprapti)

• Samprapti Ghataka (Components):
  • Dosha: The somatic doshas (Vata, Pitta, Kapha) and the psychic doshas (Rajas and Tamas).
  • Dushya (Tissues): Manas (Mind) and Sanjna (Consciousness).
  • Adhisthana (Site): Hridaya (Heart) and Manovaha Srotas (Mental channels).
• Pathogenesis:
  1. Due to unwholesome food and improper conduct, the somatic Doshas get aggravated.
  2. Simultaneously, the mind is afflicted by Rajas (passion/agitation) and Tamas (inertia/darkness).
  3. These Doshas move upwards and enter the Hridaya (the seat of the mind).
  4. They obstruct the Manovaha Srotas (channels carrying mental impulses).
  5. This obstruction clouds the intellect (Buddhi) and disconnects the mind from reality, resulting in Unmada.

5. Symptoms (Lakshana)

• General Symptoms:
  • Vibhrama: Confusion of the intellect and mind.
  • Abaddhavak: Incoherent or irrelevant speech.
  • Paryakula Drishti: Agitated or unstable eyes.
  • Aruchi: Loss of appetite or dislike for food.
• Prodromal Symptoms (Early Warning Signs):
  • Ringing in the ears (Karnasvana).
  • Dreaming of being intoxicated, sinking in water, or dancing with demons.
  • Urticarial rashes and anxiety.
• Advanced Symptoms:
  • Hallucinations: Seeing non-existing things (like darkness or shapes) and hearing non-existing sounds.
  • Loss of social conduct and modesty.

6. Different Types or Classification (Bheda)

Ayurveda classifies Unmada into five main types based on the Dosha involved:

1. Vataja: Characterized by excessive movement, incoherent speech, emaciation, and wandering.
2. Pittaja: Characterized by anger, threatening behavior, burning sensation, and aggression.
3. Kaphaja: Characterized by lethargy, silence, lack of appetite, and withdrawal.
4. Sannipataja: A severe combination of all three Doshas.
5. Agantuja (Exogenous): Caused by external factors like shock, grief, or curses (interpreted as severe psychological trauma).

7. Diagnosis

• Ayurvedic Diagnosis (Roga Pariksha):
  • Darshana (Observation): Assessing behavior (laughing/crying without reason), eye movements (unsteady/fixed), and physical signs.
  • Prashna (Questioning): Checking for memory loss (Smriti bhramsha), orientation, and dietary history.
• Differential Diagnosis:
  • Apasmara (Epilepsy): While both involve the mind, Apasmara is characterized by fainting, foaming at the mouth, and falling, whereas Unmada is primarily a behavioral disturbance without necessarily losing consciousness.

8. Disease Management

Treatment involves Daivavyapashraya (Spiritual therapy), Yuktivyapashraya (Rational medicine), and Satvavajaya (Psychotherapy).

Panchakarma (Bio-Purification)

• Vamana (Emesis): Indicated for Kaphaja Unmada to clear heavy toxins and mucus blocking the channels.
• Virechana (Purgation): Recommended for Pittaja Unmada to remove excess heat and bile.
• Nasya (Nasal Administration): Administering herbal drops (like Siddharthaka oil or Mustard oil) to "wake up" the mind and clear the head channels. This is crucial for mental disorders.
• Shirodhara/Shirovasti: Pouring medicated liquids on the forehead is highly effective for calming the mind, treating anxiety, and managing psychological disorders.
• Anjana (Collyrium): Applying stimulating pastes to the eyes to restore consciousness and mental clarity.

Medicines (Ghritas and Formulations)

• Kalyanaka Ghrita: A famous medicated ghee formulation specifically indicated for Unmada, epilepsy, and memory loss. It balances the Tridoshas and improves mental faculty.
• Mahapaishachika Ghrita: Improves intellect and memory and is used to treat mental disorders.
• Lashunadi Ghrita: Garlic-processed ghee used for mental disorders involving Vata.
• Siddharthaka Agada: A formulation used for bathing, snuffing, and anointing to clear mental cloudiness.
• Old Ghee (Purana Ghrita): Ghee stored for over 10 years is considered a specific remedy for neurological and psychological disorders.

Diet (Ahara)

• Sattvic Diet: Fresh, wholesome food to promote mental clarity (Sattva).
• Avoid: Incompatible foods (Viruddha Ahara), alcohol, and spicy/tamasic foods.

9. Prognosis

• Sadhya (Curable): If the disease is of recent onset, the patient has good physical strength, and the cause is a single Dosha.
• Asadhya (Incurable): Sannipataja Unmada (involving all three Doshas) is difficult to treat. Signs like constant wakefulness (insomnia), fixed gaze, and complete loss of physical strength indicate a poor prognosis or imminent death.

10. Modern Correlations

• Schizophrenia: Vataja and Kaphaja Unmada share features with schizophrenia (hallucinations, withdrawal, disorganized speech). Modern reviews explicitly link Unmada treatments to Schizophrenia management.
• Psychosis: The Ayurvedic description of "loss of contact with reality" and "perversion of intellect" mirrors the clinical definition of psychosis.
• Therapy: The use of Panchakarma (detoxification) and Shirodhara (oil flow therapy) is recognized in integrative medicine for managing psychiatric conditions.
