Classics kept medicine for early timira and declared mature cataract surgical - the honest scope of Ayurvedic care.
Cataract is where Ayurveda's eye science is at its most impressive - and most honest. The classics staged the disease precisely, treated the early stage medically, and declared the mature stage surgical. Here is the real account.
Sushruta's Uttara Tantra (the eye-ENT section) describes a graded vision-clouding disease of the pupil's inner layer: timira - early misting with the pupil still normal-looking; kacha - deeper clouding with the pupil changing color; linganasha - the fully clouded, whitened pupil (modern mature cataract). The staging even notes the pupil-color sequence - a clinical observation level that matches modern slit-lamp descriptions. Ashtanga Hridaya's Uttara Tantra and Madhava Nidana carry the same framework.
For early timira: netra tarpana (medicated ghee bathing of the eye), anjana (eye salves), oral Triphala ghrita and vata-pitta pacifying regimens - nourishment-and-protection care for a lens still mostly clear. This maps reasonably onto "early stage: protect, nourish, slow."
The honesty line: the classics declare linganasha - the mature stage - a condition for surgical intervention, not medicine. Sushruta describes the couching procedure (displacing the clouded lens from the visual axis) with pre-operative preparation, the technique, and post-operative care instructions, and grades which cases are operable. A 2,000-year-old text refusing to promise medicine for a mature cataract is the tradition at its most credible.
The couching procedure is history - modern phacoemulsification removes the clouded lens through a tiny incision and implants a clear artificial lens (IOL), with 15-20 minutes, local anesthesia, same-day discharge, and among the highest success rates in all of surgery. The classical principle (mature cataract = surgical problem) survives; only the tools improved beyond recognition.
Any drops, ghee preparations or courses claiming to dissolve a mature cataract - classical-branded or otherwise - are selling delay. The classics themselves would refuse that claim.
Sushruta's Uttara Tantra (ch. 17) and Ashtanga Hridaya describe couching (linganasha vedhana) with a surgeon's precision: a copper needle (tamra shalaka, ten angulas long, bud-tipped); entry at the natural soft spot half an angula from the corneal limbus while the patient looks toward the nose; the opaque lens pressed gently downward until the pupil clears and the patient confirms they can see; then warm ghee, a soft bandage, and days of flat rest in a dark draft-free room, forbidden to cough, sneeze or strain. The instructions to operate only on the fully mature, firm, pearl-white (kaphaja) cataract - and the list of types never to touch - read like an ancient fitness-for-surgery checklist.
The classics staged cataract as timira-kacha-linganasha, treated the early stage medically, and declared the mature stage surgical - exactly the modern line; anyone selling drops that "dissolve" mature cataracts is contradicting both systems at once.
