Bipolar I, bipolar II, cyclothymia and rapid cycling explained - how the types differ, why bipolar II is not "lite", and the classical Unmada patterns.
"Bipolar" is a family of related conditions, and the type matters - it shapes treatment, prognosis, and how urgently episodes must be handled. The types are defined by how high the highs go.
At least one full manic episode (with or without depression):
Hypomania plus major depression - never full mania:
Two or more years of fluctuating hypomanic and depressive symptoms that never reach full episode criteria - a chronic, lower-grade instability that still impairs and still deserves treatment; a fraction progresses to bipolar I or II.
The classical Unmada classification by dosha dominance - Vata (agitated, flighty), Pitta (angry, intense), Kapha (withdrawn, slow), plus mixed and grief-triggered types (Charaka, Chikitsa Sthana 9) - is the tradition's way of saying "these illnesses come in distinguishable patterns needing distinguishable care." It does not map one-to-one onto bipolar I/II/cyclothymia; the modern typing is the clinically decisive one, and the classical lens is a complementary way of describing temperament of episodes.
Which type is most serious? Bipolar I carries the most dangerous highs (psychosis, hospitalization); bipolar II carries comparably serious depressions and suicide risk. Both are serious and very treatable - "which type" guides therapy more than it grades danger.
Can bipolar II turn into bipolar I? Yes - if a full manic episode ever occurs, the diagnosis becomes bipolar I permanently. It's one reason ongoing review matters.
What is the difference between hypomania and happiness? Happiness is proportionate and flexible. Hypomania is sustained (4+ days), comes with reduced need for sleep, pressured speech and uncharacteristic risk-taking, and is often followed by a crash.
Is cyclothymia worth treating? Yes - years of low-grade instability erode work and relationships, and treatment (psychoeducation, rhythm regulation, sometimes medication) genuinely steadies it.
