Wheeze, night cough, chest tightness and breathlessness that come in episodes - the asthma pattern to recognise, the signs of poor control, and when to call 112/102.
Asthma's hallmark is that symptoms come and go - a person can feel completely normal between episodes, which is exactly why so many people underestimate it. The classic four are wheeze, cough, chest tightness and breathlessness, and the pattern matters: symptoms that vary, that worsen at night, and that respond to triggers point strongly to asthma.
What makes these symptoms asthmatic is their variability: they come in episodes, worsen at night and early morning, flare with triggers (dust, cold air, smoke, exercise, infections), and ease - fully or partly - with a reliever inhaler or on their own.
Poor control is not "just my asthma" - it means the preventer treatment needs stepping up, and it raises the risk of a severe attack.
Call 112 or 102, or reach an emergency department at once, if an attack brings:
While waiting: sit upright, loosen tight clothing, take reliever puffs as per your asthma action plan, and do not lie down.
The classical picture of Tamaka Shwasa reads like a bedside description from today: attacks of loud wheezing (Ghurghuraka), violent coughing fits, momentary relief after coughing up sticky phlegm, breathlessness that makes lying flat impossible so the patient finds relief sitting upright (Asino labhate saukhyam), cold sweat on the forehead, darkened vision in severe spells, and attacks worsening in cloudy or rainy weather - Madhava Nidana, Chapter 12; Charaka Samhita, Chikitsa Sthana 17. The observation that sitting upright helps is exactly what modern emergency guidance still advises.
Can asthma exist without wheezing? Yes - cough-variant asthma presents with cough alone, typically at night or after exercise. A persistent night cough lasting weeks deserves spirometry, not just cough syrups.
How is asthma different from a chest infection? Infections cause fever, coloured phlegm and a continuous illness that resolves in days to weeks. Asthma causes recurrent episodes without fever, with completely normal gaps between them.
Why are my symptoms worse at night? Airways naturally narrow in the early morning hours (circadian rhythm), reflux worsens lying down, and dust mites live in bedding. Night symptoms are also a key marker that control is slipping.
Is breathlessness during an attack dangerous if I have had it before? Every severe attack is dangerous, regardless of past experience - asthma deaths typically happen in people who had "survived worse before." Treat the emergency signs above as non-negotiable.

Exercise and yoga improve asthma control - with the preventer inhaler in place. Safe practices, real evidence, and the one rule: nothing replaces the reliever in an attack.
