Abrupt high fever, severe symmetric joint pain, mid-illness rash, deep exhaustion - and the dengue overlap that must be tested.
Chikungunya announces itself abruptly - a well person in the morning can be bed-bound by evening. The symptom cluster is distinctive once you know it.
Muscle aches, eye redness or pain behind the eyes, swollen lymph nodes, and in children sometimes a more flushed, blistering rash. Appetite collapses; exhaustion dominates.
Both arrive the same way (same mosquito, same season). Clues favoring chikungunya: joint pain is severe and early, platelets usually stay near normal. Clues favoring dengue: bleeding, falling platelets, plasma leakage. Because early dengue can masquerade as chikungunya, doctors avoid aspirin and ibuprofen until dengue is ruled out - those drugs raise bleeding risk in dengue.
Bleeding from gums or nose, black stools, persistent vomiting, severe abdominal pain, confusion, breathlessness, chest pain, reduced urine, a bulging soft spot or refusal to feed in an infant - these suggest dengue or complicated disease, not simple chikungunya.
Abrupt high fever with severe symmetric joint pain (wrists, ankles, fingers), a mid-illness rash and deep exhaustion - distinct enough to suspect, overlapping enough with dengue to test.
